Acting on the basis of the recommendations of the World Health Organization (WHO) on the announced pandemic COVID-19 coronavirus, the provisions of the Act of March 2, 2020 on special solutions related to the prevention and eradication of COVID-19, other infectious diseases and crisis emergencies caused by them (Journal of U.2020.374) and of the Resolution No. 42/03/VII/2020 of the Management Board dated 10.03.2020 on extraordinary measures to limit the risk associated with COVID-19 infection at Centrum Techniki Okrętowej SA, this health questionnaire is introduced.
